Voluntary Participation: I am voluntarily electing to undergo the treatment(s) indicated above at Royal Beauti Bar. I understand the nature and purpose of the selected treatment(s).
Risks and Discomforts: I understand that all aesthetic and body contouring treatments carry potential risks, including but not limited to redness, swelling, temporary discomfort, minor bruising, skin sensitivity, or other unforeseen reactions. I understand that results may vary and are not guaranteed.
Medical History: I have disclosed all relevant medical history, including any conditions such as pregnancy, skin disorders, heart conditions, or any other medical concerns that may contraindicate the selected treatment(s).
Consultation and Information: I have had the opportunity to ask questions about the treatment(s) and have received satisfactory answers. I understand the post-treatment care instructions and agree to follow them as directed.
No Medical Treatment: I understand that the services provided by Royal Beauti Bar are non-medical and cosmetic in nature. They are not intended to diagnose, treat, or cure any medical condition.
Liability Waiver: I release Royal Beauti Bar, its owners, employees, and contractors from any liability for injury, loss, or damage that may arise as a result of my participation in these treatments, except where caused by gross negligence.
Photographic Consent (Optional): I grant permission for before-and-after photographs to be taken to document my treatment progress. These photos will be utilized unless I provide written notice instructing otherwise.
Cancellation Policy: I acknowledge that cancellations or rescheduling within less than 12 hours may result in a fee, as outlined in Royal Beauti Bar’s policy.
Agreement to Terms: I understand and agree to the terms of this waiver and release.
